Locating your 1199a form on the AFG web site.

Step #1: Log into the application website at https://portal.fema.gov/famsVuWeb/home

Step #2: Look to the right side. The 1199a form is under this drop down menu.

Step #3:  To see your 1199a form you need to go to the first year that you got an award. It is under that year.

Step #4:  Open the form and look in the upper left hand corner of the form
